จากภาพจะเห็นว่า epithelial cells ทุกชั้นมีการเปลี่ยนแปลงแบบ dysplasia ( top-to-bottom change) โดยยังไม่มีการลุกลามลงสู่ connective tissue stroma ข้างใต้ การเปลี่ยนแปลงแบบ dysplasia ที่พบในระดับเซลล์ได้แก่ hyperchromatism (การติดสีจัดของ nucleus) , disorientation of cell arrangement (การเรียงตัวของเซลล์ไม่เป็นระเบียบ) , mitotic figure (เซลล์เกิดกระบวนการแบ่งเซลล์) , individual cell keratinization (มีการสร้างเคอราทินภายในเซลล์)้
